Assertive Community Treatment

Our Assertive Community Treatment Team (ACTT) includes medical, behavioral health, and rehabilitation professionals who work together to provide person-centered care. Services are delivered in the home and community, not just in an office setting. This team approach helps individuals who have frequent hospitalizations, difficulty maintaining employment, or challenges managing daily responsibilities.

ACTT provides individualized treatment, medication support, skill building, housing support, vocational assistance, and family support. The goal is to help individuals become more independent, stay stable in the community, and achieve their personal recovery goals.

Referrals for ACTT services may be sent to ACTTreferrals@daymarkrecovery.org. Referrals are accepted from treatment providers, hospitals, physicians, LME-MCOs, and community members.

ACTT Helps Individuals Who

Are age 18 or older with schizophrenia, bipolar disorder, or other psychotic disorders

Have difficulty completing daily tasks needed to live independently

Struggle to maintain employment or stable housing

Have frequent psychiatric hospitalizations or crisis service use

Have ongoing mental health and substance use challenges lasting longer than 6 months

Have recent involvement with the legal or justice system (arrest, incarceration, probation)

Have not improved with traditional office-based outpatient treatment

ACTT services begin with a comprehensive clinical assessment to make sure the program is the right level of care. The ACT Team serves as the main provider of services and offers frequent, community-based support with a small staff-to-individual ratio. Care is flexible and adjusts over time as a person’s needs change.

Services are available 24 hours a day, 7 days a week, 365 days a year, with weekday office hours for calls and walk-ins. The goal is to help individuals spend less time in hospitals and become more independent in their community.

ACTT is designed for individuals with severe and persistent mental illness and is not intended for those whose primary diagnosis is substance use disorder, intellectual or developmental disability, borderline personality disorder, traumatic brain injury, or autism spectrum disorder.
